INTENSIVE WAIVERED SERVICES
Affinity Caregiver is a licensed provider of 245D Intensive services, including Individualized Home Support with Family Training and Individualized Home Support with Training.
BASIC WAIVERED SERVICES
Affinity Caregiver is a licensed provider of 245D basic services including Individualized Home Supports Without Training (IHS), Homemaker, Respite, Night Supervision, and Individual Community Living Supports (ICLS).
HOUSING SUPPORT
We are dedicated to providing personalized care and access to housing for all individuals. With supportive housing and resources for those experiencing long-term homelessness, we are able to meet the needs of our clients.
EMPLOYMENT SERVICES
Individualized services and supports that help people maintain paid employment in community businesses/settings.
HOUSING STABLIZATION
Our services includes comprehensive housing transition and stabilization assistance. We collaborate with community resources to offer medical assistance benefits that aid people with disabilities and seniors in locating and securing housing.
EMPLOYMENT SERVICES
Individualized services and supports that help people maintain paid employment in community businesses/settings.
CFSS (COMMUNITY FIRST SERVICES & SUPPORTS)
CFSS will expand people’s choices about how they receive their services, including who can provide services,
